A healthcare website is two projects wearing one name. There is the public marketing site, which behaves like any other website, and there is everything that touches patient information: intake forms, portals, appointment scheduling, anything wired into an electronic health record. The second half carries HIPAA obligations, a signed Business Associate Agreement and an accessibility standard most commercial sites ignore, and it is where an agency that has not done this before gets into trouble.

The agencies below split along that line, and so do the buyers. This page ranks fifteen of them on evidence you can check, says which of three quite different shortlists you are actually on, and reports what each firm publishes about compliance and about cost. Which, in this category, turns out to be remarkably little.

Transparency usually carries 15 to 20. It carries 5 on this page because not one healthcare specialist publishes a price, a band or a rate, and a criterion nobody scores on sorts noise rather than firms. Documented outcomes takes the difference, because this is one of the few categories where agencies publish real numbers against named organisations: appointment volumes, scheduling revenue, portal registrations. Recognition stays low because the awards in healthcare marketing are mostly paid entry.

What these fifteen publish

The most useful thing to know before you start calling, because it is where the field separates, and it separates in a direction you would not guess.

  • The specialists publish nothing on price. Modea, Geonetric, Reason One, OHO Interactive, Kanopi Studios and Eastern Standard have no pricing page, no budget field on the contact form and nothing in their sitemaps. The rule holds in reverse too: the generalists with healthcare work mostly do publish, from $10,000 at Halo Lab, Perpetual and Creative Corner Studio, through $25,000 at Finsweet and Viget and $30,000 at Netguru, to $50,000 at Verndale.
  • Two put a compliance commitment in writing. Finsweet states that where any part of a project involves it handling patient information, it signs a Business Associate Agreement before work starts, and it separates the Webflow marketing site from the systems that handle that information. Kanopi Studios states that every medical site it builds is HIPAA-compliant and meets at least WCAG 2.1 AA. Nobody else on this page commits to either in public.
  • Three attach numbers to named health systems. Modea reports 418 percent growth in online appointment scheduling at Ballad Health and a 57 percent increase in scheduling revenue at Luminis Health. Reason One reports 107 percent more urgent care appointments at Lee Health. These are the agencies' own published claims rather than audited figures, but each one names the organisation, which most of this category does not.

    What separates it from the other specialists is that it owns software. VitalSite, released in 2007, is its own healthcare content management system, and since 2023 it has also partnered with Optimizely on a healthcare digital experience platform. Which one, given what you are

A hospital or health system replacing a legacy site. Modea or Geonetric. Both work on nothing else, they name more than thirty health systems between them, and both have done the electronic health record work before rather than promising to learn it.

Several sites to consolidate into one. Modea, which reports doing exactly that at Luminis Health: more than twenty sites onto one platform with a shared design system, and Epic MyChart wired into the result.

A children's hospital or a cancer centre. Reason One, on Children's Nebraska, SickKids, Golisano and Moffitt. That is the most concentrated record of its kind here, and accessibility is a standing commitment rather than a line in the proposal.

An academic medical centre that also needs a brand. Eastern Standard. Almost no healthcare web specialist does identity work and almost no branding agency has the healthcare record, so the overlap is small and this is in it.

A health tech or digital health product. Finsweet. It is the only firm here that says in public where the line runs, and a company building a clinical product is the buyer most likely to need that line drawn correctly.

A clinical-stage or life sciences company. Netguru for scale, with Merck, MilliporeSigma and Polpharma on the record, or Perpetual, which took Alto Neuroscience's brand and site through a funding round.

Neither is a healthcare specialist, and what you are buying at this level is a good website from a team that has done one or two healthcare projects, which for a marketing site with no patient data in it is often exactly right.

A single practice or a clinic group chasing appointment volume. None of them, and it is worth saying so plainly. That is a healthcare marketing problem rather than a web design one, and the firms built for it sell paid search, local SEO and call tracking with the website attached. Ask for appointment volume as the deliverable rather than a site, and you will find yourself talking to a different set of companies.

Settle this before you sign

Ask which parts of the build will handle patient information and what specifically happens to those parts. It is the question that separates the firms on this page, and only one of them has answered it in writing where you can read it before the call. Everyone else will answer it well or badly in a meeting, and you want that answer in the contract rather than in the pitch.

How much does a healthcare website cost?

It depends which half of the project you mean, and almost nobody in this market will tell you before a call. Every one of those figures comes from a generalist. None of the six healthcare specialists publishes a price, band or rate anywhere on its site, so a health system redesign has to be scoped in conversation. A useful shape: a marketing site with no patient data in it is priced like any other website, and anything involving portals, intake or an electronic health record integration is a different and larger number.

Does a healthcare website have to be HIPAA compliant?

Only the parts that create, receive, store or transmit protected health information. A marketing site describing your services and listing your clinicians usually does not, and a patient intake form, a portal, an appointment request that captures symptoms or anything wired into an electronic health record usually does. This is why the better agencies map the project before designing it, separating the parts that touch patient information from the parts that do not, and build them differently. An agency that treats the whole project as one undifferentiated build has not thought about it.

What is a Business Associate Agreement and do I need one from my agency?

A Business Associate Agreement is the contract that makes a vendor handling protected health information on your behalf legally accountable for protecting it. If your web agency will touch patient data at any point, including during testing or migration, you need one. Ask when in the process it gets signed. Do I need Epic or MyChart integration?

Healthcare sites also carry a harder version of the problem than most: patients using them are disproportionately likely to have a disability, be older, or be reading under stress, so conformance and genuine usability come apart more than usual. How long does a health system website redesign take?

Consolidating multiple hospital or service line sites into one adds months at the front, in content inventory and governance rather than design. Ask for the timeline in writing beside the estimate, ask what has to be true for it to hold, and ask which of those things depends on your side rather than theirs.
